- When you go to get your permit or license, you will need to be accompanied by a parent, guardian, or Authorized Driver Training Instructor. They will need to bring their ID and sign the application. If they are your legal guardian, they will also need to bring documentation showing that they are your legal guardian.
- If they are not your parent or guardian, they will need to sign a Responsible Adult Affidavit, provide their ID, and submit paperwork to show your relationship. Examples include:
- School Documents
- Tax Information
- Military Documents
- Driverβs License, permit, or ID card
- Employee ID if you are signing the application as the responsible adult and you are an employee of the agency that has custody of t00hat minor - for example, a social worker or employee of a homeless shelter.
- If you are a step-parent married to biological parent not listed on the minor's birth certificate, you will need to:
- Sign a responsible adult affidavit at the CSC stating your relationship to the minor and
- Provide state-issued ID or Letter from State agency to verify your identity and
- Submit paperwork to show your relationship (example: marriage certificate or license, school documents, tax information, military document)
- If you are the biological parent of the minor but your name isn't on the birth certificate, you will need to:
- Sign a responsible adult affidavit at the CSC stating your relationship to the minor and
- Provide state-issued ID or Letter from State agency to verify your identity and
- Submit paperwork to show your relationship (example: school documents, tax information, military document)
- If you're the biological parent of the minor but do not wish to submit their birth certificate to the DDS:
- Your minor will need to submit a document to verify his or her identity. This may be a birth certificate or US Passport.
- In addition to that identity document, you must also:
- Sign a responsible adult affidavit at the CSC stating your relationship to the minor and
- Provide state-issued ID or Letter from State agency to verify your identity and
- Submit paperwork to show your relationship (example: school documents, tax information, military document)
- Minor who are not required to have a Responsible Adult sign their application:
- An emancipated minor
- A minor who is married
- A minor on active duty with the armed forces of the United States
